If the use of recombinant DNA in this research is limited to constructs which can only be propagated in E.coli K12, yeast, or similar cell types (plasmids, bacterial, yeast artificial chromosomes), and it DOES NOT involve the use of native or recombinant mammalian viruses, potentially infectious agents, biological toxins, or transgenic animals, it does not require approval from the WSU Institutional Biosafety Committee (IBC).

If this research DOES involve the use of native or recombinant mammalian viruses, infectious agents, biological toxins, or transgenic animals, it requires review and approval by the WSU Biosafety Committee (IBC).
IBC approvals are valid for three (3) years from the initial review/approval process. Approvals apply only to the lab originally inspected and the investigator originally approved. After three years you must submit a new BAUA form.
Amended / Updated Protocols: If this research is identical or very similar to a protocol approved by the IBC within the past three years, you may submit an amendment or update:

New Protocols: If this research has never been reviewed by the IBC, or it has been more than 3 years since its approval, complete and submit all relevant forms below. The first page of each form asks for the same information. If you are filling out more than one form you only need to complete the first page and the "Principal Investigator's Agreement" ( last page) once.
All labs working with non-exempt agents must also provide a copy of standard operating procedures (SOPs). If you already have a SOP for this experiment, you may send it in with the application form. To develop a SOP you may use this document:
